Home care has become an integral part of the health care system as patients have shorter hospital admissions and are discharged to their homes requiring more care and skilled services. Moultonborough Visiting Nurse Service works closely with hospital discharge planners to ensure the necessary services are in place in a timely manner when patients return to their homes. We also work closely with patient families and other significant caregivers as they play a major role in the patientís ability to remain at home.A plan of care is established based on the patientís physicianís orders. Although our goal is to assist patients to remain in their home as long as possible, there are times home care is unable to adequately meet the patientís needs. The home care nurse will assist with appropriate referrals when this occurs.
Services available to the patient:
Skilled Nursing:
Our nursing staff provides a comprehensive assessment to determine the patients home care needs. A plan of care is established under the direction of the patientís physician. The nurse may provide wound care, medication teaching, and injections, assess cardiac and pulmonary status, provide teaching of the diabetic patient, etc.
Occupational Therapy:
The role of Occupational Therapist is to assist individuals to gain functional independence with daily life such as: bathing, dressing, meal preparation, and leisure activities.
Physical Therapy:
The role of Physical Therapist is to evaluate and treat patients with muscloskeletal dysfunctions by means of therapeutic and strengthening exercise, balance exercise and gait training, Range of Motion, and establish a Home Exercise Program.
Speech Therapy:
The Speech-Language Therapist is to evaluate and provide treatment for patients with speech and swallowing disorders.
Medical Social Service:
The Medical Social Worker is to assist the patient and care-givers with long
range planning, financial resources and referrals t community resources.
Home Health Aide:
The Home Health Aide assists patients with personal care, dressing, meal preparation and Home Exercise Program established by a therapist.
